COVID-19 Testing Portal

For the safety of our program participants, staff, and volunteers, NCCJ requires COVID-19 tests for overnight programming. Please use this form to submit verification of your test result.

NCCJ allows COVID-19 home tests or proctored tests at a testing center. Please select the below instructions for at-home or proctored testing and read them in their entirety.

Testing at a proctored site

Schedule the appointment with the company you will be testing at (Kroger, Walmart, CVS, Etc.)

For a PCR test, you must have the test administered after within 72 hours of your programs scheduled drop-off time.

For a Rapid test, you must have the test administered within 24 hours of your programs scheduled drop-off time.

Tests administered before the allowable time window will not be allowed.

Once you receive the test results (either via paper or email), upload a copy to the form below.

At-Home Testing

At home tests can be found for free at the Greene County and the Dayton Metro Libraries.

You may also request free home tests at https://www.covid.gov/tests

We recommend getting your home test ASAP and storing it until your program begins.

Administer your COVID-19 test per the test instructions. If you are under 18, a parent or guardian should oversee your test and verify the results. Once your test results are complete, take a photo of the test and upload it using the below verification form.

At-home tests must be taken within 24 hours before your scheduled program drop-off time.

Please Note: Rapid tests must be administered within 24 hours of the start of your program, and PCR tests within 78 hours. Tests administered before this date will not be allowed.
